The Queensland state coroner today handed down his findings on Hamid Khazaei's death and surprise, surprise, it was preventable. www.thecourier.com.au/story/5554330/asylum-seeker-death-preventable-coroner/?cs=7Hamid Khazaei, 24, died at Brisbane's Mater Hospital in September 2014 after an ulcer on his left leg became infected at the Manus Island detention centre. He suffered multiple cardiac arrests before being evacuated to the Brisbane hospital, where he was pronounced brain dead. In his findings handed down on Monday, coroner Terry Ryan said "a series of clinical errors, compounded by failures in communication" led to significant delays in him being transferred to Port Moresby and eventually Australia. "Mr Khazaei's death was preventable," he said. "I am satisfied that if Mr Khazaei's clinical deterioration was recognised and responded to in a timely way at the (Manus Island) clinic, and he was evacuated to Australia within 24 hours of developing severe sepsis, he would have survived." Mr Ryan said those involved in Mr Khazaei's health care were "well intentioned", but that his treatment was not in line with Australian health care standards. He said it was crucial the Australian government improve the care given to asylum seekers in offshore centres. He made a series of recommendations, including that the federal government develop and implement a written policy relating to medical transfers, and said the approvals process should be led by people with clinical training in emergency medicine. He further recommended that the Commonwealth Attorney-General establish laws to ensure all deaths of asylum seekers in refugee camps be independently investigated in Australia.

How sad (a needless death).
You don't muck around with sepsis..although initially it can be hard to diagnose.
I hope the Govt enforces the Coroner's recommendations.

They won't. The want the asylum seekers/refugees in offshore detention to "disappear" and don't give a shit about their wellbeing. They have been systematically wearing them down both physically and psychologically for over 5 years now and have no intention of stopping.

I had just finished reading that when I logged in here. Heartbreaking. Ava, I don't think they actually want them to disappear, I think it's even more cynical than that. They want their lives to be so visibly and excruciatingly awful that it acts as a deterrent to others.

The government continues to deny the awfulness and always will. Dutton tells lies about the conditions on Manus and Nauru almost on a daily basis. Dutton is a cruel and arrogant man who has been given too much power. He wants them all to be gone. Death or refoulement are the two main options. The people trafficking exercise with the USA is an option for some.
The Australian public is largely shielded from what really goes on in offshore detention, largely because of the mainstream media being silenced. You won't read about the women being woken in the middle of the night by male guards, you won't read about the Hindu men whose only choice of meat is beef. You won't read about the children, happily packing their belongings after being told by the guards that they will be going to the USA when it is already known that they have been denied. You won't read about the truly abhorrent things that have happened and will continue to happen. The boats have not stopped. They never have. Dutton and his cronies now admit this. There have been reports of bodies being retrieved from the sea after boats that have been towed back have subsequently sunk.
It is all being documented and those responsible will be brought before the ICC.

The AFP are liaising with the DPP about whether charges should be laid over leaks from Michaelia Cash office about raids on the AWU.

It's shocking that young man died simply from stepping on something and cutting his foot AC
It's common knowledge that a lot of money has "gone missing" from border force, or Dept. of Home Security or whatever Dutton calls himself

Developing countries are not known for their medical expertise. Hamid died as a consequence of mistake after mistake after mistake after mistake after mistake was made medically, and, because Australian bureaucrats made decisions that dismissed the urgency of Hamid's rapidly deteriorating condition.
He should have been transported from Manus to Australia when the DIPD was notified. He would not have died. If both the Manus clinic and Port Moresby Hospital had equipment that actually worked, he might have had a chance. He was hooked up to a ventilator that was faulty .the alternative bag had a tear in it so was useless; the drip was not properly inserted; he was not intubated earlier when he should have been. The ICU at Port Moresby Hospital is on a different floor to the ED. There is no internal lift and access is via a narrow and steep exterior ramp. The ramp was inaccessible due to rain. It just goes on and on, and, gets more and more depressing.
This is how we treat those who turn to us for safety.

On Friday night, a Somalian refugee collided with another soccer player and damaged his knee. [He is in the East Lorengau Transit Centre which has a clinic now run by PIH who also run the hospital in Port Moresby] The clinic was already closed for the weekend and there is no doctor on duty at Lorengau hospital at night [Lorengau is the main town on Manus. Our government donated a new world class hospital to Lorengau] On the Saturday, he went to the hospital. Eventually, he was given pain relief and told to come back on Monday for an xray. On Monday, the PIH doctor told him to go to the hospital for an xray and then to come back to PIH so he went to Lorengau Hospital. There, he was told that an xray was not possible because the xray machine was broken and there was no one on Manus who could fix it. So, his knee was bandaged then he went back to PIH who said that they can't help him until he has an xray. He can't get an xray because the machine is broken.

A quick update on Abdi. He finally had his knee xrayed today and his patella tendons have snapped. He is being flown to Port Moresby tomorrow morning for urgent surgery - one week after the injury occurred.

With only panadol and a bandage for pain relief. Appalling. There is also a man who lost sight in one of his eyes during the attack by guards and locals that resulted in Reza Barati being murdered. He is now losing the sight in his other eye and without urgent medical care, he will soon be blind. The treatment that he needs is not available on Manus. He tried to commit suicide last week and was saved by his friends.
Our government pays PIH {Pacific International Hospital] nearly $5m per month for them to provide medical care to the men.

Dutton is a cruel and heartless man.. it's not the first time the court has ruled against what he wants
I'glad Sara Hansen Young is suing David Leyonhjelm.. that mam seems to be an idiot
all the stories going on about that female politician... Emma Husar, seem a bit far fetched to me, many are saying it's a distraction from the GBRF scandal that the press isn't reporting on

Abdi is still on Manus with no news of when he will be flown to Port Moresby - hopefully, tomorrow. The doctor on Manus has refused to give him an injection for the pain. His patella has been torn away from the lower leg.

Abdi is finally on his way to Port Moresby. Hopefully, he will be get the medical care that is needed and that it isn't too late. He is just one of many who the public has had to fight for. There have been so many, still are so many, and will continue to be so many until this government of ours is removed from office. It shouldn't be this way.
Dutton hired a lawyer to stop a very sick baby from being transported here for urgent medical treatment. Yesterday, Dutton was overruled, again, and the baby along with his parents are expected to arrive in Sydney sometime over the weekend.
